本園已經開放115學年度新生入學登記確認，麻煩家長接到電話通知後，或已經來電登記入園者，請至以下表單填寫幼生相關資料以完成報名程序。

新生入園當日請準備以下物品(開學日為8/3星期一)：
1、戶籍謄本或戶口名簿影本(請勿省略記事欄）
2、兒童預防接種紀錄影本
3、準備濕紙巾、衛生紙、香皂、室外拖鞋、午休寢具、三組餐具(三色碗及餐具袋)、水壺、2-3套備用衣物、備用口罩、其他個人用品(例如：尿布、防蚊液、擦手巾)
4、本園有代售餐具組(三色碗附提袋)、書包、透氣棉被袋及運動服，若需要購買，可於入園後再向老師登記訂購。
5、幼兒的衛生紙及濕紙巾用畢後，班級老師會再通知家長補充。
6、請為孩子在個人用品 (包含外套) 寫上名字。
7、繳費方式於入園後再開立，請家長收到繳費單後再行繳費。
Enrollment confirmation for new students for the 2026 academic year (Year 115 in Taiwan’s calendar) is now open. Please prepare the following items on the first day of school (Monday, August 3):
A copy of the household registration transcript or household registration booklet (please make sure the remarks section is included).
A copy of the child’s vaccination record.
Please prepare the following daily items:
Wet wipes
Tissues
Soap
Outdoor slippers
Nap bedding
Three sets of tableware (three-color bowls and a utensil bag)
Water bottle
2–3 sets of spare clothes
Spare masks
Other personal items (e.g., diapers, mosquito repellent, hand towel)
The school provides tableware sets (three-color bowls with carrying bag), school bags, breathable quilt bags, and sports uniforms for purchase. If you need any of these items, you may register with the teacher after enrollment.
When the child’s tissues or wet wipes are used up, the class teacher will inform parents to replenish them.
Please write your child’s name on all personal belongings, including jackets.
Payment instructions will be issued after enrollment. Please make the payment after receiving the payment notice.
